HalALL-Star 3on3 Women Basketball Tournament

This form is for registering your 3 on 3 basketball team for the HalALL-Star Basketball Tournament on August 12th, 2017 at 11:00am. Please complete all the fields with accurate information. You will also be required to agree with the waiver published below. Please keep in mind all teams must have participants with the same gender. It is preferred to have 5 players registered in case players are unable to make it.

To participate in the “HalALL-STAR WEEKEND 3 ON 3 BASKETBALL TOURNAMENT”, related events and activities, the undersigned acknowledges and agrees that:

1) The risk of injury from the activities involved in the program is significant.

2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume all full responsibility for my participation.

3) I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

4) I hereby release and hold harmless "HalALL-Star Weekend Basketball Tournament," their officers, officials, agents, other participants, sponsors and if applicable, owners and lessors or premises used to conduct the event.

5) FULL PAYMENT MUST BE RECEIVED BEFORE A TEAM WILL BE OFFICIALLY REGISTERED. NO EXCEPTIONS WILL BE MADE.

With respect to any and all injury, disability, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, I have read this release of liability and assumption of risk agreement and fully understand its terms. I understand that I have relinquished substantial rights by signing it and sign it freely and voluntarily without any inducement.
