Hi, thank you for your interest in booking a tattoo with me!
I'm currently booking for SEPTEMBER & OCTOBER 2025!

I work part time from my private studio in East Van nicknamed The Stabshack.
I am available Sundays, Mondays and Tuesdays and Wednesdays. I work single session days and occassionally double for smaller pieces.

If you have any questions or would like to chat before submitting this form you can reach me at bugonline@proton.me. I ask for deposits to secure tattoo appointments with me of $100.00CAN which can be sent via e-transfer to a email address I will give you upon replying to your intake form.

GUIDELINES..

**!PLEASE READ THRU BEFORE SUBMITTING THIS FORM!**

Be over 18 and vaccinated for COVID. Masks are mandatory, N95's, KN95's or any decent, well fitted and mask with a good seal. No single ply surgical masks please.

Cash only for payment.

If you won't be able to tip, please let me know in advance so I can manage my books accordingly.

Please read over the tattoo portion of my website and make sure you’re familiar with how I work before reaching out.

In the event of needing to cancel within 72 hours of our session or no showing, I will keep your deposit. I may turn down rebooking no shows that are without good reason.

If you're more than half an hour late the deposit will become non deductable from final cost. If I don't hear from you within an hour of our appointment time, I will consider our appointment cancelled.

Please do not come see me if you're sick! Let me know as soon as possible.

If you have frequent risk exposure to COVID in your life, please test twice before our session. Suggested 7 days and 42 hours beforehand.

Ask before photographing me or taking photos with me in them. I will make sure to do the same for you and your tattoo.

Understand that I get to turn down work I can't, am not able or do not want to do.

I ask that all clients be mindful of COVID transmission, exposure and risks around the time of our appointment and commit to bringing and wearing high quality, properly fitted masks in session with me.

I also ask that if you have frequent risk exposure to COVID in your life, that you test twice before our session. Suggested 7 days and 42 hours beforehand. (Ex: non-masking, irregular masking, busy life, work or family settings etc)

In my own life, I am covid aware, masking in all indoor public spaces, in crowded outdoor spaces and most interpersonal personal settings. I rapid testing when needed and use additional mitigations. If you have any COVID specific questions or requests I am very open to accomodations. I am high risk due to multiple health conditions and being on immunosupressant medication. I have been continually vaccinated for COVID and keep up on boosters.
