To prevent the spread of COVID-19 (and any other diseases) and reduce the potential risk of exposure to our workplace and visitors, we are conducting a simple screening questionnaire. Your participation is important and required to help us take precautionary measures to protect you and everyone in the studio. Thank you for your time, consideration, and truthful responses.

This agreement is between Diamond Studio Photography, hereinafter referred to as the “Photographer”, operating in Canada, Ontario, and the signers of this agreement. The acceptance of the booking is based on agreement by the client of the terms and conditions stated herein. By booking with Diamond Studio Photography, you enter into a contract and have therefore agreed to abide by the terms and conditions.

You agree to reschedule if you cared for someone diagnosed with COVID-19 or any other contagious viruses and/or diseases within the 14 days of the session day.*

You agree to reschedule if you been out of the country or have been in contact with someone diagnosed with COVID-19 or any other contagious viruses and/or diseases within 14 days of the session day.*

You agree to reschedule if you experienced any cold or flu-like symptoms within 14 days of the session day.*

I am not asking my clients to wear a mask. You can wear it only if you wish so. You understand that the Photographer is not responsible for your personal decision and agree to not blame the Photographer or her staff if you get sick after the session. You also agree to sanitize your hands as you enter prior to touch anything.*

You understand that the photographer may not wear the mask. If you feel uncomfortable with this decision, please let the photographer know about that. She will wear a mask then. You understand that the photographer has difficulty breathing in the mask during the photo session. Sometimes the photographer needs to bring the mask down for a while, but will keep physical distance minimum of 1 meter between the photographer and clients. (Especially during the newborn/milestone sessions, when the room temperature is set to high degrees to keep the studio warm for babies/children)*

You understand that you and/or your family members can't wear a mask during the photo session. Due to that fact you and/or your family members are exposed to the virus (COVID-19 or any other contagious viruses and/or diseases).*

You understand that the Photographer cleans and disinfects the studio in the best way she can.*

You understand that You are signing for yourself and for all other family members who will attend the photo session, including, but not limited to, your children/grandchildren, husband/wife etc.*

You agree to follow good respiratory hygiene by covering mouth and nose with elbow or tissue when cough or sneeze. And to dispose of used tissues immediately and clean hands.*

You understand that if you are not taking pictures and the child is adult enough to remain alone in the studio, you will wait in the hall, in the waiting area.*

You understand that everything from the above applies to the outdoor/on location photo sessions.*

By signing this agreement, I acknowledge the contagious nature of COVID-19 or any other contagious viruses and/or diseases and voluntarily assume the risk that I and/or someone from my family members may be exposed to or infected by COVID-19 by my mere presence within this establishment and that such exposure or infection may result in personal injury, illness, permanent disability and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions or negligence of myself and others, including, but not limited to, employees, volunteers and other clients and their families. I hereby release the booked business from any and all claims arising from or in connection with any direct COVID-19 impact while visiting.
