Kindness, Respect, and Dismissal Policy
Canadian Girls Baseball is a child-serving organization, and we're here to make sure that our participants have the best experience possible! As a grassroots, non-profit organization that is actively growing, we encourage and welcome your feedback as parents and caregivers, and we appreciate your patience and understanding as we continue to grow.
While constructive, kind, and respectful feedback is always welcome, we will not tolerate any communication that is aggressive, disrespectful, condescending, or rude. Engaging in this type of communication may result in your child's dismissal from our organization, which would be very sad.
We operate from a model of kindness, respect, and dignity toward all children and families that we serve, and we expect this to be reciprocated in all communications and interactions with directors, staff, coaches, and volunteers.
Medical Release and Authorization
As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.
Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.
Permission is also granted to Canadian Girls Baseball and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.
Release authorized on the dates and/or duration of the registered season.
This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
Canadian Girls Baseball encourages parents/guardians to remain near the field during the duration of the program.
This release(s) and indemnifies CGB and its directors, officers, partners, staff, employees, consultants, representatives, from any and all claims and liabilities whatsoever arising from participation in or attendance at one or more of CGB's current or future programs by the undersigned, the undersigned’s child(ren) or any associated spectator(s); the undersigned acknowledges that participation in CGB's programs involves a voluntary assumption of all the risks associated with the sports and games being played.
Permission is granted by the undersigned for Canadian Girls Baseball and its agents to use above named child's video and likeness for purpose of promotion by CGB in all forms, including but not limited to news releases, photographs, video, audio, marketing, advertising, trade, promotion, exhibition, for an indefinite period of time. The undersigned gives unrestricted permission for images, videos and recordings of the child to be used in print, digital and internet media. The undersigned acknowledges that there will be no compensation for the images usedand waives any right to inspect any printed or electronic copy. I hereby release CGB assigns of any claims that may arise from these uses.
