Nominee Eligibility:

1. You must be an undergraduate student at SFU.
2. You must be a member of the SFSS.
3. You must be 18 years or older, OR have the signature of a parent if you are under 18.
4. You must sign the SFSS statement of consent and release form.

Terms and Conditions:

1. If you are in more than one faculty, you can only be nominated for one.
2. All nominees must be notified prior to the submission of the application, and provide both their signature and student number as consent.
3. Late or incomplete applications will not be accepted.
4. The application must not contain defamatory or discriminatory language.
5. Nominees can nominate themselves.
6. All nominees must comply with SFSS Bylaw 2; “be a member in good standing of the SFSS”. This statement means a student who satisfies the requirements of By-Law 2 and has paid all fees, fines and penalties levied in accordance with these By-Laws or the Society’s regulations. This statement is NOT a reference to academic standing with Simon Fraser University. Students not currently enrolled in classes may pay the SFSS membership fee to remain a member in good standing.

I, the aforementioned nominee, hereby consent to the disclosure of any and all information required to the Simon Fraser Student Society (SFSS)  Women of the Year Awards Selection Committee, by Simon Fraser University (SFU) for the purposes of confirming my eligibility to be nominated as a candidate for the SFSS Women of the Year Awards. My consent expires on April 30th, 2020. No use or disclosure of personal information about me to the SFSS by SFU is permitted beyond the terms described herein without my express written authorization or unless permitted or required by law.

I, the aforementioned nominee, do hereby give the Simon Fraser Student Society (SFSS), their assigns, licensees, successors in interest, legal representatives, and heirs the irrevocable right to use my (a) name (or any fictional name), image, voice, and (b) all photographs, recordings, videotapes, audiovisual materials, writings, and statements, in all forms and in all media and in all manners, without any restriction as to changes or alterations (including but not limited to composite or distorted representations or derivative works made in any medium) for advertising, trade, promotion, exhibition, or any other lawful purposes, and I waive any right to inspect or approve the photograph(s) or finished version(s) incorporating the photograph(s), including written copy that may be created and appear in marketing materials produced by the SFSS.
