    Alzheimer's disease and other Dementias are progressive and typically progress through stages, which are commonly described as “early”, “middle”, “late”, and “end of life”. Everyone living with dementia is different and may experience each of the stages differently and at different rates. Join us as we discuss some of the common things a person with dementia may experience as their dementia progresses, and helpful supports through the stages.

    Advanced Care Planning is the process of thinking about your wishes and directions for healthcare and end-of-life situations and communicating those wishes to the person/people who will make decisions for you if you are not able.

    This webinar will help provoke thought and conversation about the importance of Advanced Care Planning. We will explore the role of a Substitute Decision Maker / Power of Attorney and look at tools for creating your Advanced Care Plan. Dr. Rhonda Collins will also briefly discuss Medical Assistance in Dying (MAID).
