Thank you for applying with Abundant Life Home Health Agency. With our Fast-Track hiring program, you can start work sooner. In this step, you will upload all the required documents to start work with our agency.

We require the following documents:

• Driver's License
• Social Security Card
• Proof of Auto Insurance
• ActiveFlorida Nursing / CNA License
• CPR Card
• Most Recent CEU/ Transcript
• Photo of Yourself
• TB Test / Chest X-Ray Report 
• Physical Exam Report 

If you do not have one or more documents, please let us know why, and when you can submit them in the comment boxes provided. Remember, all documents must be in before you can start work.

• Log on to CEbroker.com
• Click on "log in".
• If you already have an account: enter username/password
• If you do not currently have an account, click “Create account” (you do not have to pay for the professional account- create a free account and you will have access to the information)
• Click on "my records" tab at the top of the page. This is a drop down menu. Click on "course history".
• Right click on page and select print. Save as PDF. Upload in the space below

The report must include a statement that you are free from communicable disease, and any health impairment that it a potential risk to the patient, or may interfere with the performance of your duties.
