Submission of this form is a request to have an athletic therapist work your game(s).Athletic Therapists that are assigned could be Sheridan Students (3rd or 4th year), graduated but un-certified therapists, or fully certified athletic therapists.If you wish to have the AT tape your players prior to the game, the expectation is that you provide the AT with the necessary supplies.
COSTS

Single Game - $85 (Football $120)

Double Header Game - $120 (Football $185)

Triple Header Game - $160

Tournaments/Mini Tournaments - Hourly Rate - $37/Hour

All rates are +HST if applicable.

AT must have an HST # on their invoice to charge HST.
